EFT Cancellations

EFT Cancellation Policy
To cancel your EFT Membership we MUST have a completed EFT cancellation form (or signed letter) by the 15th of the month PRIOR to the month that you would like to cancel. Phone cancellations are not accepted.

Example: If you would like to cancel or put your membership on hold for February 1st then we must have a completed form in our possession no later than January 15th.

NOTE: We will be unable to refund any monies if the EFT Hold or Cancellation form was not received by the 15th of the month. Please refer to your EFT agreement for more information.

Completed forms can be dropped off at the center during regular class hours.
